Ampicillin 50 - 100 mg/kg/dose Q 12 hours iv
Gentamicin Loading dose for everyone: 4 mg/kg iv Q 24 hours X 1
Maintenance dose:
Gestational age: > 37 4 mg/kg iv Q 24 hours
30 – 36 3 – 3.5 mg/kg iv Q 24 hours
< 29 2.5 mg/kg iv Q 24 hours
Gent. levels: in term infants generally trough and peak around 4th dose (if any suspicion of poor renal function check trough before 1st maintenance dose). In premies (<2500 g) check trough and peak around 2nd dose (1st maintenance dose)
Cefotaxime: 50 mg/kg/dose Q 12 hours (use sparingly, e.g. in renal insufficiency)
